Consumer Health Data Privacy Policy
This Consumer Health Data Privacy Policy applies to “Consumer Health Data,”
as that term has been defined under applicable law. Any term appearing in
this Consumer Health Data Privacy Policy (the “Policy”) that has been
defined under applicable Consumer Health Data laws shall have the meaning
afforded to it therein.
Collection of Consumer Health Data
We Collect the following categories of Consumer Health Data from Consumers:
- Individual health conditions, treatment, diseases, or diagnosis;

Precise location information that could reasonably indicate a consumer's
attempt to acquire or receive health services or supplies;
- Data that identifies a Consumer seeking health care services;

Health-related data that have been derived or inferred from the above.
We Collect and use the above-mentioned categories of Consumer Health Data
for the following purposes:

provide you with the relevant products or services (for instance to
deliver a prize you may have won in a contest).

We may combine any Consumer Health Data we collect about you with any other

We may also deidentify any Consumer Health Data we collect about you. When
we do so, we take reasonable measures to ensure that the information cannot
be associated with a consumer or household, and we maintain and use the
information in deidentified form. We will not attempt to reidentify the
information, except that we may attempt to reidentify the information solely
for the purpose of determining whether our deidentification processes
satisfy applicable legal requirements. After it has been deidentified, the
information is no longer Consumer Health Data and is not subject to this
Policy.
The Categories of Sources from Which We Collect Consumer Health Data
We may collect Consumer Health Data directly from consumers in a variety of
contexts, including: (1) via signups on our Services, such as through the
creation of an account; (2) from an online, email, retail, fax, or telephone
purchase; (3) when you enter a sweepstakes, giveaway, contest, or other
promotion, or complete a survey; (4) when you provide information at our
stores; (5) when you contact us, such as through customer service
communications, including our online chat features; (6) when you participate
in an event we host or attend; (7) when you register a Company product; (8)
when you submit a business reply, product, or warranty card; (9) when you
post material via our Services, such as through product reviews; or (10)
when you interact with us for any other purpose.
We may also collect Consumer Health Data from other sources, including from
vendors and third parties when they provide Consumer Health Data to us, and
from cookies and other tracking technologies (collectively, “Cookies”) used
on our Services. If you provide delivery information or other Consumer
Health Data which is not your own to us, then you must have that person’s
permission to give us their Consumer Health Data and for us to use and
disclose it as provided in this Policy.
The Categories of Consumer Health Data that We Share

Entities with Whom We Share Consumer Health Data
We Share Consumer Health Data with the following entities:

affiliates: Stanton Optical, Physicians Eyecare Group and OpticallBPO.

To our Vendors. We disclose the Consumer Health Data we collect to
our vendors who act on our behalf. For example, we use vendors to operate
our Services; host our websites; and to help us with our promotional
efforts. These vendors may need Consumer Health Data about you to perform
their obligations. We contractually require our vendors to keep the
Consumer Health Data they process on our behalf confidential and to use it
only to provide services on our behalf.
